For all children entering Georgia schools for the FIRST time,
the following vaccines must be documented on certificate #3231:
DTP series
HIB series
Polio series
Hepatitis B series
1 Chickenpox or history of disease
2 MMR's (or 1 MMR and 1 Measles)

All sixth graders will be required to present an immunization
certificate showing the date of their chickenpox vaccine
OR the date they had chickenpox disease.
